From 1st July 2025, Department of Health is changes some guidelines for chronic disease management access in the clinical setting.
What is “MyMedicare” Registration?
MyMedicare is a voluntary patient registration system that allows individuals to nominate a preferred clinic or doctor to manage their primary healthcare needs.
MyMedicare Vs Medicare Card?
Medicare Card: A Medicare card is a healthcare identity card issued to Australian citizens and permanent residents. It allows individuals to access a range of medical services and benefits under Australia's public healthcare system, Medicare.
MyMedicare: MyMedicare is a voluntary patient registration system where individuals nominate a preferred general practice or medical centre for their ongoing care. To register for MyMedicare, individuals must hold a valid Medicare card and have an existing relationship with the nominated medical centre.
What are the benefits of MyMedicare Registration?
MyMedicare-registered patients can access Chronic Disease Management Plans, which include up to five subsidised allied health sessions per calendar year. (Up to $300 towards allied health consultations).
By registering, patients form a stronger partnership with their chosen clinic—leading to more coordinated care and better health outcomes. Our dedicated nursing team will stay in touch to provide ongoing support in managing chronic conditions.
When necessary, extended telehealth consultations are also available to MyMedicare-registered patients for added convenience.
Eligibility: a Medicare card or DVA card holder, at least TWO face to face visits recorded with us in the previous 24 months.
